When ti follows a vowel, it becomes cc

ti > ty > cc

e.g. ati > aty > acc + anta > accanta

In Kaccāyana this is called (19) sabbo caṃ ti

and (28) para dvebhāvo ṭhāne.

         
ati anta accanta adj final, complete
iti etaṃ iccetaṃ pron thus this
iti āsi iccāsi aor it was thus
ati āyata accāyata adj too tightly stretched
ati āraddha accāraddha adj overly aroused
pati atta paccatta adj personal

as you can see this mostly occurs with the indeclinable iti and the prefixes ati and pati
